Support Coordinator

Our client is a family friendly organisation, that encourages and supports clients as well as their staff.  Their services provide compassionate, professional and experienced support workers who are passionate about the service they provide.

They provide respite care, house cleaning, domestic assistance and personal care whilst supporting NDIS goals.

They are looking for a Support Coordinator with at least 2 years experience who has had success working alongside clients, their families and other support providers.

Responsibilities:
  • Collaborate with participants and their support networks to plan how their funds will be allocated and identify the services and supports required.
  • Guide and assist providers to align their services with each participant’s goals and funding, fostering quality outcomes and genuine collaboration.
  • Work alongside participants to identify and connect with suitable supports that best match their needs, enabling them to actively pursue their goals and aspirations.
  • Research, coordinate, and manage a range of opportunities across multiple providers to meet individual support needs.
  • Demonstrate consistently high standards in both written and verbal communication, with strong and comprehensive report-writing skills essential.
  • Effectively budget and allocate participant funding to maximise the value and outcomes of each participant’s plan.
Required:
  • Working with Children's Check or NDIS Worker check
  • Police check (or willing to obtain)
  • Drivers Licence
  • Experience as a Support Coordinator or similar, working within the NDIS sector
  • Certificate III in Community Service or Disability
